Insulin Titration Protocol
!
BG < 150 mg/dL
STOP insulin โข Add dextrose โข Monitor closely
โก
BG 150-250 mg/dL
Continue insulin โข ADD dextrose support โข Monitor closely
โ
BG 200-250 mg/dL
Continue current CRI โข Maintain dextrose if needed
โ
BG > 250 mg/dL
Continue CRI โข No dextrose โข Reassess in 2-4 hours
Core Calculations
Concentration = Total Units รท Bag Volume
Units/Hour = Weight ร Dose (U/kg/hr)
Pump Rate = Units/Hour รท Concentration
Units/Hour = Weight ร Dose (U/kg/hr)
Pump Rate = Units/Hour รท Concentration
โฐ Recommended Monitoring
Blood Glucose
Every 2-4 hours
Electrolytes
q4-6h (Kโบ, Naโบ, Clโป)
Acid-Base
BG < 250 or q6-12h
Ketones
q12h or per protocol
๐ก Clinical Pearls
- Target BG decrease of 50-75 mg/dL/hr initially
- Add dextrose when BG reaches 250 mg/dL
- Monitor for hypoglycemia and hypokalemia
- Phos and Mg often drop during recovery
